Summary: Can only book an appointment via PATCHS

We are pushed to booking online only by the Patchs system. I have taken 40 minutes to fill the form in for it to be rejected. It shouldn't be so difficult to book a GP appointment, I was in a lot of pain at one point, went to the surgery and was told to fill in a Patchs form, I couldn't was in so much pain.
They push you toward booking online because you've used it before. I have had to ring 111 to try and get an appointment and have been told to come in and fill in Patchs form.
I am not a digital expert and you should be able to book over the phone or at the surgery, why is it so difficult to get an appointment?
