My wife has to put blood pressure readings in once a month. The practice pharmacist will then discuss medication and then the physician associate alters the thyroid medication. I'll say to them, have you spoken to the endocrinology team about this and they'll say no. There is no continuity of care at the GP surgery, they don't understand her medical history, and there is no communication between the practice and the specialist teams that care for her.
