Difficulty getting a clear answer about which organisation holds responsibility for records when care is delivered partly by the practice and partly by an out-of-hours provider. Requests for my own records met with a partial response, late, and without explanation of what had been searched or what was withheld. Protracted correspondence needed to get basic procedural answers.
